New Patient Registration
To register at the practice, please complete the registration form below. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
To help us locate your existing medical records it is really important that you complete your NHS number on this form. You can obtain this from your previous GP. It is not your National Insurance number.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice. gms1.pdf
Our practice has a specific registration area, a detailed area map can be viewed here. If you move but still in our practice area we can continue to be your GP.
Although sending information across the Internet means that privacy of data cannot be absolutely guaranteed,
our online forms offer high levels of security. The forms use HTTPS and TLS (using a 2048 bit certificate) to
encrypt data sent to and from your computer when using the site and our server environment is PCI DSS certified -
the highest standard of security associated with e-commerce and online finance solutions.
Form submissions are stored on the server for 28 days and are encrypted and secured with a private key known only by the GP Practice.
