New Patient Registration
Please first check that you are living inside our catchment area
To register at the practice, please complete the Pre-registration form (also known as a GMS1 form) and the Online Medical Questionnaire For New Patients. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
To help us locate your existing medical records it is really important that you complete your NHS number on this form. You can obtain this from your previous GP or by visiting the NHS Website. It is not your National Insurance number.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
The practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance.
You may print off the Pre-registration form, fill it out and bring it in with you on your first visit to the practice.
