GP Online Services
As a registered patient with Chatfield Health Care you can now book GP appointments and request repeat prescriptions via your computer, smartphone or tablet at a time convenient to you rather than calling or visiting the surgery, thanks to the Patient Access service.
GP Online services can save you a phone call or journey to the surgery, and allow you to access these services when you need them even when the surgery is closed. GP Online services work alongside traditional methods of accessing your GP services, it does not replace them. By allowing people who want to make appointments and order prescriptions in this way it should become easier for patients who may find it difficult and frustrating trying to get through to the surgery on the phone during busy periods.
If you have a long-term health condition you can additionally benefit from GP Online services as they can enable you to have greater involvement in your healthcare by providing you with access to your test results and helping you to keep track of your treatment via access to your online medical record.
If you would like to register for GP Online services you will need to complete an application form, which is available via reception. We are also required to verify your identity and you will need to bring 2 forms of ID with you, one of which should have a photo (such as a passport or driving licence) and the other which should include your address (such as a driving licence, council tax bill or bank statement but not a utility bill). If you don’t have photo ID or anything with your address on it, this doesn’t mean you will not be able to use GP Online services, so do not hesitate to ask our surgery staff who will be able to help.
Your completed application form must be approved by a GP. Please note that this may take up to 2 weeks to process. If your application is successful you will be informed via post or email and will receive a document with your Practice ID and PIN, which you will need in order to register online via Patient Access (via the link below).

You can view, book and cancel your GP appointments online.
PLEASE BE ADVISED THAT PATIENTS ARE CURRENTLY UNABLE TO BOOK OR CANCEL APPOINTMENTS ONLINE. UNFORTUNATELY WE HAVE HAD TO DISABLE THIS SERVICE DUE TO AN IT ERROR OUTSIDE OUR CONTROL, WHICH IS CURRENTLY UNDER INVESTIGATION BY OUR IT CLINICAL SYSTEM SUPPLIER. WE WILL ENDEAVOUR TO GET THIS SERVICE UP AND RUNNING AS SOON AS POSSIBLE. WE SINCERELY APOLOGISE FOR ANY INCONVENIENCE THIS MAY CAUSE.
If you are on repeat medication (this does not include controilled drugs) you may request your presriptions online. The prescription will be ready to collect two working days later.
You can ask your chemist or the surgery to set up electronic prescribing to send your prescription electronically to your preferred chemist in any location. There are also local chemists who will order, collect your paper prescription and deliver your medication. If you wish that your prescription be returned to you by post, please provide a stamped addressed envelope.
Please be aware that on some occasions you may need to see your doctor for a review before a further prescription will be issued.

Your Personal Medical Record
You may be granted access to some or all of the following information held in your electronic medical record:
- test results (coded data only)
- problems (coded data only)
- consultations (coded data only)
Whilst every endeavour is made to ensure your GP computer records are accurate, if you do find any infomation that is incorrect please contact the surgery and speak to the duty doctor as soon as possible.
